How to Apply


To apply to be an appointed attorney representing indigent criminal defendants during initial appearances:

  1. Complete the application
    • Word
    • PDF (fillable) Note: If you want to save the information you enter into this form on your computer, you must have Adobe Acrobat or other software. If you do not have Adobe Acrobat or other software, use the Word version. The Word version allows you to save the form and the information you enter into the form on your computer.
  2. Complete the Anti-Discrimination Acknowledgement Form
  3. Complete a Form W-9 (Request for Taxpayer Identification Number and Certification).
  4. Submit the completed application, Anti-Discrimination Acknowledgement Form, and Form W-9 in one of the following ways:
    • Email the completed application, Anti-Discrimination Acknowledgement Form, and Form W-9 to appointedattorneys@mdcourts.gov
    • Mail to:
