Please be advised that the Complaint/Application and Affidavit in Support of Judgment (DC/CV 1) form has been revised. The "Notice to Defendant" on the back of the form was revised to match the notice on the Assigned Consumer Debt Complaint (DC/CV 106).

The revised form is available online through the link in this directive and in our online Forms Index. The printed forms will be available at our warehouse in approximately 6-8 weeks. Please wait until late November before ordering the new version. All filers MUST begin using the new 11/2012 (or 10/2012) version by January 1, 2013. Thank you for your cooperation.
