About the Forms
The Maryland Judiciary is improving access to justice by providing court forms as a FREE service to the public. These forms are under continual revision. We recommend that you check the revision date on the FORMS INDEX to ensure you have the correct version.

Forms

These are forms for individuals who do not have a lawyer and need to file papers in one of the following areas: divorce, custody, visitation, child support, alimony, name changes or contempt.

PETITION FOR PROTECTION FROM DOMESTIC VIOLENCE
Form used to file for protection from domestic violence, child abuse or vulnerable adult abuse.

CASE MANAGEMENT
For use by court staff only. These include scheduling orders and form orders for a variety of family-related services and procedures.

JUDGMENTS & ORDERS
For use by court staff only. These include form judgments and orders for use in simple domestic cases and CINA cases.
