Change of Name or Address Procedures

Please follow the information below to make changes to your address or to your name with Human Resources, Central Payroll Bureau (CPB) and Health Benefits. 

If you work in a Circuit or District Court, send the original forms to your local HR Liaison
For AOC and District Court Headquarters, please send the forms to Human Resources.

Employees who have a legal name change or address change must follow the instructions below:

  • Change of Name or Address
    • Follow the link below.
    • Under the category “Other,” select “Update Personal Information for Regular. Employees” https://mdcourts.gov/hr/connectupgradetipsheets
    • Follow the instructions on the Tip Sheet. If this is for a name change, see the section Name Change below. If for an address change, proceed to the section Address Change below.
  • Name Change
    Note: Before the name change is finalized in CONNECT, you must submit a copy of your Social Security Card with new name and proof of legal name change:
    • Marriage - copy of your marriage license signed by the Clerk of Court
    • Divorce - copy of divorce decree
    • Name Correction - copy of the Court order

    • Address Changes
      If making changes to your address in Connect, please also follow instructions to log into POSC to update your address with Central Payroll.
      • In Connect from My Page select tile “Pay Data.”
      • Select “Central Payroll Online Service” from the left margin, Or
      • Visit https://interactive.marylandtaxes.gov/Extranet/cpb/POSC/User/Start.aspx. To create an account, our Payroll Staff, 410-260-1735, can provide you with your latest Advice Number.
      • Once logged in, you will land on the welcome page
      • Click “Update Address / W4 (Tax Withholding)”
      • Click on “Change W-4 / MW507information”
      • Make any necessary changes to each section and click ‘Next’. The various sections are:
        • Section 1 – Employee Information
        • Section 2 – Federal Withholding Form W-4
        • Section 3 – Maryland Withholding Form MW 507
