- Choose the form for the State you will be living in during your clerkship. There are separate forms for:
- Maryland, DC, and West Virginia
- If you reside in Pennsylvania or Virginia you still use the Maryland form, but in Section 3, complete question 4
- On the W-4 Form complete the following and write legibly.
- Section 1 – Payroll System – RG – Regular
- Agency Code: 220100
- Employing Agency: State of Maryland – Judiciary
- SS Number
- Name
- Address (Important: your payroll checks will be mailed to this address, until your direct deposit takes effect)
- County of Residence: Please enter your county as the default will be Harford County, which has the highest tax rates.
- Section 2 – Federal Taxes – Complete line 3; and then either line 5 or line 7
- Section 3 – State Taxes -- Marital status and then line 1, or 3, or 4, or 5
- Line 4 is only for law clerks who will reside in Pennsylvania or Virginia during their clerkship (check the box and then certify with “exempt” on line 4)
- Section 4 – Sign and date the form
- Section 1 – Payroll System – RG – Regular
If you make an error, you may cross through it and enter the correct information; however, you must initial the correction. No white out is allowed on the form.
